This form does not replace other mandatory reporting requirements.
- For all patient saftey incidents an SLS must also be lodged.
- For Work Health and Saftey Incidents an IRQA should be lodged and notification to the SAAS SDM (1800 886 268).
- Child abuse or neglect reports can be lodged at the DCPs website or by calling 13 14 78.
