Membership coverage, for volunteers engaged in such paid contract/casual work, will only extend to employment matters whilst they are so engaged.

Authorisation of Payment / Deductions
I hereby authorise you to pay to THE AMBULANCE EMPLOYEES’ ASSOCIATION OF SOUTH AUSTRALIA hereinafter called “the Union” of which I am a member, the sum of 1.36% (D992) of the base rate per fortnight (hereinafter called “the subscription”) out of any wages or salary due or becoming to me by the Government of South Australia, provided:
1. That the authorisation may be cancelled by me upon my revocation, in writing to the Union, of this authority.
2. That where the rates of subscription are varied by the Union, in accordance with its rules, the amount of subscription authorised herein may be varied accordingly upon receipt by you of notice in writing from the Union of such variation.
3. Should I cease to be a member of the Union I shall notify the Union and the relevant payroll section of the future date from which this authorisation shall cease.

Conditions
1. This form must be personally signed by the employee applying for membership.
2. Deductions will occur only where a copy of this form, duly signed, is forwarded to the relevant payroll section.
3. The Union may advise the relevant payroll section or a relevant Department(s) of a variation in the rate payable by way of subscription.,
4. The Union is to comply with Department procedures regarding deduction of membership subscriptions.
5. Payroll/the relevant Department is to pay the Union every two weeks or every four weeks, as agreed, and must supply a reconciliation of total subscriptions deducted where required by the Union.
6. Applications for membership will be referred to the Union’s State Council for determination as to admission pursuant to the Union’s Rules.
7. Membership of the Union entitles members to assistance only in relation to employment which is declared to the Union in writing (on this application form or otherwise) and where the Union is in receipt of subscriptions in respect of that employment.
8. The Union may only assist a prospective member in relation to an issue which arose prior to the date of this application if, and to the extent that, such assistance is authorised by State Council. Authorisation may be provided on conditions or with limitations, including (but not limited to) a requirement to pay a service fee.
9. These terms and conditions are supplemented by and incorporate the following:
